Nutrition Screening
Three steps, scored 0–2 each; sum the scores.
> 20 = 0 · 18.5–20 = 1 · < 18.5 = 2
< 5 % = 0 · 5–10 % = 1 · > 10 % = 2
Interpretation
| Total | Risk | Action |
|---|---|---|
| 0 | Low | Routine clinical care; repeat screening. |
| 1 | Medium | Observe; document intake for 3 days; repeat. |
| ≥ 2 | High | Refer to dietitian. Set goals, improve and increase nutritional intake, monitor. |
A two-step screen. Initial screen first — if any answer is Yes, complete the final screen.
Initial screen
Final screen
Interpretation
Final score = nutritional status + severity of disease + age bonus. A total ≥ 3 indicates the patient is nutritionally at risk and a nutrition care plan should be initiated.
